The following forms are required information for registration. Please assist us by having them completed upon arrival:
OUSD Emergency Card - (1 per student)
OUSD Injury & Insurance Waiver (English) - (1 per student English or Spanish)
OUSD Injury & Insurance Waiver (Spanish)
OUSD OTC Authorization Form - (1 per student)
PTA Membership Form - (1 per family)
PTA Annual Student Waiver - (1 per student)
